Waist measurements are taken flat from one side to the other.

Therefore:

12" flat relaxed waist = approximately 24" around
24" flat stretched waist = approximately 48" around

The elastic waistband is designed to accommodate different waist measurements; 48" represents the approximate maximum stretched garment circumference, not the recommended body-waist measurement for a comfortable fit.

The shirt has an intentionally oversized drop-shoulder construction, so a conventional fitted chest measurement is not the best indicator of fit.
